After trumpets of the apocalypse proclaim the fall of humanity, vampires arise from the shadows to rule the earth. Yūichirō wants just one thing—to get revenge by killing each and every vampire.
Yūichirō and his friends regroup to plot out their next steps in the ongoing war between the humans, vampires, and the Hyakuya Sect, and time is not on their side. Shinoa fights for her life to keep the First trapped inside of her. But the First has allies on the outside. With Krul now on Shinoa squad's side, they may actually have a chance of winning and saving Mika. But first, the gang learns the truth of what happened between Krul and Mika eight years earlier.

Volume 23's cover features Shinoa Hīragi as a child sitting while wearing a large white t-shirt, and dark colored shorts, with nothing on her feet. Staring ahead, Shinoa has her hands on her elder sister Mahiru Hīragi's arms, who is embracing her from behind while wearing a dark blue colored uniform. If the scene is depicting events in time then it would not be Mahiru-no-Yo being depicted here, as Mahiru had not fused with Noya Hienma at that point. More than likely it is a scenario involved with the flashback scene of chapter 97.
Both Shinoa and Mahiru are within a midst of golden holly leaf like foliage that appears decoratively placed below them and on either side, then apparently behind them too. It may be a golden light as to why the leaves appear this way as the berries are also golden, except for in the lower right of the image where some holly appears more as it does naturally. Golden flecks appear to be floating inwards towards the two sisters, where they also turn silver matching the moon color and light seemingly reflected on Mahiru's hair. Fully formed golden leaves float upwards on the sides.
Behind, at the top of the image is an exceptionally large silver moon backdrop. A few individual stratus like clouds float around the edges of the moon, which is notably brighter in the lower left curve of the celestial body. Around the moon there are many artificial contraptions. They are dark as with the silhouettes of the clouds, though to a lesser degree. It may very well be a representation of the medical equipment used to experiment on Shinoa in chapter 97, where the many tipped needles encircle the moon.
